Placement Partners We partner with families, caregivers, healthcare professionals, and community organizations to help seniors find safe, stable, and appropriate living communities based on their individual needs. Our placement partners assist with locating options such as: Senior living communities Personal care homes Assisted living communities Nursing homes Other appropriate residential care settings Referral Requirements To begin the placement search, please submit a facesheet that includes: Senior’s full name Current address Phone number Emergency contact and/or Power of Attorney (POA) information Diagnosis or medical conditions, if applicable Current income amount Additional documentation may be requested to help determine the appropriate level of care and support the search for a safe and secure placement. Referral Fee A $75 referral fee is provided for each referral submitted. Our goal is to make the placement process easier for families and referral partners while helping seniors identify communities that can meet their care, safety, and living needs. (please allow 24-48 hours for a follow up from your placement partner)
